Location: Account Maintenance Process/Forms Request
When you select Forms Request from the Account Maintenance process, the system displays a list of blank Forms available in the system.
To open and view a form, select the appropriate form and click View in the File ID column. In this case, the form is in .pdf format. When you click View, Acrobat Reader launches and displays the form. For example, click View beside the Address Change Form.
